Myocardial Recovery and ECMO Weaning: Who and When?

Weaning patients from extracorporeal membrane oxygenation is a delicate and sophisticated process. In this review, Eduardo Rame, MD, Medical Director of the Ventricular Assist Device Program at Penn Medicine, provides both a definition and an algorithm for successful ECMO weaning to recovery. Dr Rame also discusses a small study of critically ill peripartum patients placed on ECMO at the Hospital of the University of Pennsylvania for heart failure, with excellent outcomes for weaning, and considerations for weaning from ECMO to LVAD and transplant in patients with chronic cardiomyopathy.
